FYI, for it to work I had to change

From:
$client->scope = 'r_fullprofile r_emailaddress';

To:
$client->scope = 'r_basicprofile r_emailaddress';

Yes, since May 12, 2015, applications can only have access to full profile after they are reviewed.

So, I have changed the examples to only request r_basicprofile, although r_fullprofile would still work for approved applications.
